BRIGADE RECREATIONAL SOCCER LEAGUE

 MAIL-IN REGISTRATION INSTRUCTIONS

1.  Fill out the enclosed application as complete as possible. Parent information on top and player information at the bottom. If you are unsure, leave blank. Sign and date. If you have an e-mail, please provide it so we can communicate more effectively & efficiently. Remember, we are a volunteer organization, so please check off a volunteer position for your team. The more volunteers we have, the smoother your team and our organization runs. Please Help. We are needing Coaches and Referees. No experience necessary. We train all our Coaches and Referees and supply materials. Volunteer Coaches and  Referees could be yourself, spouse, relative, or friend that would like to help our organization, please let us know. 
2. Provide copy of birth certificate, immunization card, ID card or any document with player birthdate.
3. Provide payment in check or money order form. Payable to ‘BRSL’. $65.00 dollars per player. Please do not send cash. 
4. Put all of the above in the provided envelope. Please add your return address to envelope and provide stamp.
